Submit a PAIA Request
Under the Promotion of Access to Information Act (PAIA), you have the right to request access to any records held by Mathee Dental Studio. To submit a request, download and complete Form 2 above, then email it to us or deliver it in person. We will respond to your request within the timeframes prescribed by the Act.
